Where To Go When You Need Help
Heartside Clinic provides professional, comprehensive care
for the homeless of greater Grand Rapids. Our primary focus
is providing quality health care. We also serve as a single
point of contact for help with entitlement programs, assistance
with acquiring food and housing, and to access to the mental
health system and substance abuse treatment programs.
This continuum of services enables us to fulfill an essential part
of Saint Mary’s mission—to heal the mind, body
and spirit of our clients. All of our clients are treated
with the utmost respect and dignity.
Who We Serve
Heartside Clinic serves people who are homeless and without
a permanent residence. This includes people living on the
streets or in missions, shelters, transitional housing and
single-room occupancy hotels. Our mission also extends to
people who are doubling up with other families on a temporary
basis. Most, but not all, of our clients come from the Heartside
area of Grand Rapids, the poorest section of the city. We
average more than 600 medical and social work visits per month,
so it is clear our services are urgently needed.
Our Services
Heartside Clinic provides a range of health and social
services in one convenient location. Our nurses, social workers,
patient coordinators, volunteers, manager and physicians are
dedicated to providing compassionate, holistic health care:
- Primary health care for adults
- Physical therapy
- Podiatry
- Psychosocial assessment & care plans
- Case management
- Crisis intervention
- Mental health counseling
- Substance abuse advocacy, referral & support
- Legal advice
- HIV counseling & testing
- Assisted referrals to community social service
agencies for needs including housing, employment & mental
health services
- Assistance with application for benefits programs
- Assistance with dental care
- Shower facilities
- Bilingual English/Spanish services
Accessing Our Services
All of our services are provided on a “first come,
first serve” basis. We accept clients regardless of
their ability to pay; however, we work with clients to obtain
eligible benefits such as Medicaid or Kent Health Plan.
In addition to our regular clinic hours, a Heartside nurse
is available to address urgent medical needs 24 hours a day
by phone. Call (616) 685-5000 after hours and ask to speak with
a Heartside nurse.
The Heartside staff also works closely with other area agencies
to ensure individuals receive the right care, at the right
time, in the right place. Our partners in caring include Guiding
Light Mission, John F. Jellema Treatment Center, the Salvation
Army Adult Rehabilitation Center and various service agencies.
These outreach efforts extend our ability to help people and
increase our visibility in the community.
Who We Rely On
Heartside Clinic is funded in part by a federal grant from
the Bureau of Primary Health Care. Matching support is provided
by Saint Mary’s Health Care and the generous
donations of West Michigan individuals and organizations.
Without their help, we could not help those in need.
We also rely on the time, expertise and compassion of many
volunteers including physicians, podiatrists, physical therapists,
office staff, clinical staff and an attorney. Their dedication
has made a world of difference in many lives.
